The concept of liquid biopsy—the analysis of circulating tumor DNA (ctDNA), circulating tumor cells (CTCs), or exosomes found in a simple blood sample—is poised to revolutionize the way companion diagnostics are performed, offering a compelling alternative to traditional, invasive tissue biopsy. Tissue biopsies, while the current regulatory gold standard, are often limited by several factors: they are invasive, carry procedural risks, may not yield sufficient or high-quality tumor material for comprehensive testing, and only provide a snapshot of a tumor's molecular status at a single point in time. Liquid biopsy addresses these limitations by providing a non-invasive, repeatable, and fast method for sample acquisition. This is particularly critical in cases where tumor tissue is inaccessible, insufficient, or when monitoring disease progression or resistance during treatment is necessary.

The application of liquid biopsy as a companion diagnostic is particularly promising for real-time monitoring of patients on targeted therapies. As cancer cells develop resistance to a drug, new mutations often emerge. A liquid biopsy can rapidly detect these acquired resistance mutations (such as EGFR T790M in lung cancer) months before clinical or radiographic evidence of progression, allowing clinicians to switch therapies proactively.
